If you take a medication that costs a lot, you already know the feeling. You walk up to the pharmacy counter in January, and the number on the screen is much bigger than you expected. Then it happens again in February.
There is a program that can change how you pay for those medications, and most people have never heard of it.
It is called the Medicare Prescription Payment Plan, sometimes written as M3P. It started in 2025 and it is available with every Medicare drug plan, whether your drug coverage comes through a standalone Part D plan or through a Medicare Advantage plan.
Here is what it does.
Instead of paying the full cost at the pharmacy counter, you pay nothing at the counter. Your plan sends you one monthly bill instead. Your yearly prescription costs get spread out into steady monthly payments over the rest of the year.
You do not pay more. You pay the same total. It just arrives in pieces you can plan around, rather than all at once in the first months of the year.
For someone facing a $500 month in January, that can turn into something much more manageable.
Here is what it does not do.
It is important to us that you know both sides.
This program does not lower your drug costs. It spreads them. It does not change your coverage, your pharmacy, or which medications your plan covers. And in 2026, your yearly out of pocket prescription costs are still capped at $2,100, with or without this program.
It also is not for everyone. If your medications cost you a little each month, spreading those costs can actually make your bills feel less predictable, not more. This program helps most when your costs are high and they land early in the year.
Who this tends to help
People whose prescriptions will reach that $2,100 cap, especially when most of that spending happens in January, February and March. That often means brand name medications, specialty medications, or several prescriptions at once.
How to sign up
1) Call the member services number on the back of your plan card. Have your member ID ready.
2) Tell them you want to enroll in the Medicare Prescription Payment Plan.
3) After you enroll, the next time you fill a covered prescription you will get a monthly bill instead of paying at the counter. Pay each monthly bill to stay in the program.
You can leave the program at any time. If you do, you still owe whatever is left of what you already used.
